Privacy Policy Addition for NDIS Consent
NDIS Consent to Share Information
If you request Support Coordination services from Yamba Behaviour Support & Assessments (YBSA), you consent to YBSA completing the NDIS Consent to Share Information form on your behalf.
By providing your digital signature in our intake form, you authorise YBSA to:
✅ Complete the NDIS Consent to Share Information form using the details you provide.
✅ Submit the consent form to the National Disability Insurance Agency (NDIA) on your behalf.
✅ Act as an authorised representative when communicating with the NDIA regarding your NDIS plan and related services.
We will only share your information with the NDIS and relevant stakeholders as required for the provision of Support Coordination services.
Withdrawing Consent:
You may withdraw your consent at any time by:
